Narrative: CHIEF COMPLAINT 10.5-month well child visit; questions about bottle/formula weaning, sleep/bedtime routine, and feeding transition HISTORY OF PRESENT ILLNESS Elsie is a 10.5-month-old female presenting for a routine well child visit. Parents report she is doing well overall. She had a prolonged period of nasal congestion during her first 6 months of life that has since resolved, with the exception of one acute congestion episode in June. She had a recent episode of diarrhea that has resolved and stools have returned to normal. She is formula-fed and eating a wide variety of solid foods at home including oatmeal, banana, avocado, and eggs, as well as allergen introduction packs. Parents note that daycare continues to offer purees rather than solid foods, and they plan to request an upgrade. Parents are seeking guidance on transitioning off bottles and formula. Parents also report that bedtime has become prolonged, sometimes taking 30-45 minutes to an hour, with Elsie crying after being placed in her crib. They have been rocking and bouncing her to sleep, then placing her down. She has generally been sleeping well (approximately 7 PM to 6 AM, ~11 hours), but over the last 3 nights she has been waking and crying for 5-10 minutes before self-settling. Brief breath-holding episode after vaccinations with transient perioral cyanosis, self-resolved with brief puff to face. Weight: 19 lb 12 oz Development: Crawling. Pulling to stand on furniture. Standing in crib. Not yet walking. Making varied sounds, beginning to mimic parents. Says "dada." 7 teeth erupted. Social smile present. Appropriate stranger awareness for age. ASSESSMENT 1. Well child visit, 10.5 months — Growth is appropriate. Development is on track with crawling, pulling to stand, varied vocalizations, mimicry, and emerging social referencing with age-appropriate stranger awareness. 2. Bedtime behavioral concern — Prolonged settling at bedtime (30-60 minutes) with parental rocking/bouncing to sleep and protest crying when placed in crib. Recent brief nighttime awakenings over last 3 nights with self-resolution. 3. Formula and bottle weaning guidance — Parents seeking transition plan as patient approaches 12 months. 4. History of nasal congestion in early infancy — Resolved. 5. Recent diarrhea — Resolved, stools normalized. 6. Immunization update — Hepatitis B series incomplete (1 dose received); influenza vaccine not yet started. PLAN 1. Well child visit: Growth and development are reassuring. Safety counseling emphasized given increasing mobility — she will be exploring and placing objects in her mouth. First dental visit scheduled for next month; advised that the visit will likely be limited in scope and to avoid x-rays at this age. Dentist visits generally recommended starting around age 3. 2. Sleep and bedtime: Counseled parents on independent sleep skills. Advised placing Elsie in the crib drowsy but awake so she can learn to self-soothe to sleep.
