Narrative: SPRUCE MESSAGE VISIT Date: 2026-03-07 CC: Post-vaccination shivering, low-grade fever, and irritability HPI: Thomas presented with shivering, low-grade fever (99-100°F), and irritability the day following vaccination. His mother was concerned about the shaking, noting it occurred when he woke up in the morning and again after his nap. He was described as crabby but otherwise acting normally, with no seizure-like activity identified. The vaccination site appeared normal on inspection. Encounter: NP Catherine Watson evaluated the concern via messaging in Dr. Hobbs's absence due to illness. She reassured the mother that low-grade fever, fussiness, chills, and shivering are normal post-vaccination reactions, likely representing physiologic rigors as body temperature rises. Vaccine site was confirmed to be normal in appearance. Tylenol was recommended for symptom relief including potential achiness. Mother was asked to keep the practice updated on Thomas's progress.
