Narrative: SPRUCE MESSAGE VISIT Date: 2026-07-06 Encounter: Spruce text message followed by in-person home visit CC: 18-month well-child visit including ongoing evaluation of delayed independent walking and referral for physical therapy. HPI: Thomas was seen for his 18-month well-child check. He remained not yet independently walking at the time of the visit.
