Narrative: SPRUCE MESSAGE VISIT Date: 2023-06-10 Encounter: Spruce text message CC: Recurrent fever and loose stools while on antibiotic therapy. HPI: Jack developed a recurrent fever of 100.7°F after appearing to improve, now 6 doses into his antibiotic course. He had loose/runny bowel movements and was asking for a lot of water. Cough remained the same as at the time of the home visit (6/7). No ear pain reported, though parent noted he had not complained of ear pain previously either. Albuterol was helping the cough but made him sound hoarse afterward. Assessment: Dr. Hobbs was reassured that symptoms were not worsening. The low-grade fever and loose stools in the setting of ongoing antibiotic use were monitored. Plan was to stay the course with current antibiotic and albuterol and re-check the following day. Plan: Continue current antibiotic and albuterol. Parent advised to follow BRAT diet for loose stools. Check-in on 6/11 confirmed no fever, just cough and emotional state. By 6/12, fever returned with new concern prompting a phone call. Key clinical quotes: "I'm glad it's not worse. Is albuterol helping the cough at all?" "Ok. Let's stay the course, if you're ok with that plan and check in again tomorrow." Source: Spruce conversation t_226ST10QO3800
