Narrative: SPRUCE MESSAGE VISIT Date: 2024-03-08 Encounter: Spruce text message followed by in-person home visit CC: Sore throat with decreased appetite and subsequent vomiting, with strep circulating at school. HPI: Jack had not eaten all day and was complaining his throat had 'popcorn in it.' He was lethargic. No fever initially. Strep was reportedly going around at school. That evening, he vomited once. By the following morning (3/9), he remained very lethargic, was complaining of stomach and 'butt' pain, and had no additional vomiting but was requesting popsicles and water. Dr. Hobbs performed a home visit on 3/9. Assessment: Dr. Hobbs noted no big red flags despite vomiting and lethargy but elected to perform a home visit on 3/9 given persistent lethargy. Parent had noted one spot in the throat but the view was obstructed. Multiple illnesses circulating in the community. Plan: Keep quiet overnight on 3/8.
