Narrative: SPRUCE MESSAGE VISIT Date: 2024-09-22 Encounter: Spruce text message CC: Hand-foot-mouth disease with sores on hands, feet, and mouth. HPI: Jack developed sores on his hands and feet (foot sores causing pain with walking) and mouth sores consistent with hand-foot-mouth disease. He had been ill repeatedly in the preceding weeks. No fever mentioned at time of contact. Assessment: Dr. Hobbs confirmed the clinical presentation was consistent with hand-foot-mouth disease. Noted HFM is mostly contagious when the child has fever or loose stools; the rash itself is not highly contagious. Plan: Ibuprofen preferred over Tylenol for pain. Monitor fever and stool consistency for contagiousness guidance. Medications: - Ibuprofen OTC, dose not specified Key clinical quotes: "Ibuprofen will help any pain better than Tylenol will" "Most of he has fever or loose stools. The rash isn't that contagious" Source: Spruce conversation t_226ST10QO3800
