Narrative: SPRUCE MESSAGE VISIT Date: 2025-03-20 Encounter: Spruce text message CC: Red, swollen throat with spots; suspected strep throat. HPI: Jack had a red, swollen throat with visible spots. He was extremely thirsty, not eating, and had a very white bowel movement the night before. He had a runny nose and woke up Tuesday morning with a bad cough (though not coughing during the day). No fever. Parent suspected strep given Harry's history. Assessment: Dr. Hobbs assessed that strep was very unlikely given the absence of fever and presence of cough — both are indicators against strep. Clinical pattern consistent with a viral illness circulating in recent weeks. Dr. Hobbs requested a throat photo for further review. Plan: Monitor. Reassurance provided. Dr. Hobbs offered to bring a home otoscope/throat-viewing device to the family. No antibiotics prescribed. Key clinical quotes: "Ahhhh, very likely not strep. Seen a bunch of exactly this the last couple of weeks." "No fever and has cough are good indicators of not strep" Source: Spruce conversation t_226ST10QO3800
