Narrative: SPRUCE MESSAGE VISIT Date: 2026-08-22 Encounter: Spruce text message CC: Red, swollen eyes developing during a bike ride with possible allergic or environmental trigger. HPI: Malcolm, age unknown, went on a bike ride after lunch. During the ride, the family stopped at a friend's partially constructed garage and also used a dusty bike buggy that had been stored in the garage for approximately a year. By the end of the ride, Malcolm's eyes were red and began swelling. Benadryl was administered at home. Breathing remained normal. Parent questioned whether this was a typical allergic reaction or something more serious given the swelling. Assessment: Allergic/irritant ocular reaction most likely triggered by dust from the stored bike buggy and/or construction site allergens. Breathing normal, which was reassuring against systemic anaphylaxis. Plan: Benadryl already given by parent — appropriate. Shower recommended to remove any residual allergens/irritants from skin. Ice on eyes for swelling. Monitor breathing. Medications: - Benadryl Standard OTC dosing, given once at home by parent (new) Key clinical quotes: "how's his breathing?"
