Narrative: SPRUCE MESSAGE VISIT Date: 2025-07-31 Encounter: Spruce text message CC: Stomach aches since starting guanfacine 1mg twice daily. HPI: Miles had recently restarted guanfacine at 1mg twice daily following the medication change discussion. The parent reported he was experiencing stomach aches, a known side effect of guanfacine. Miles appeared somewhat tired but was playing and acting normally otherwise. No vomiting, severe pain, or fever was described. Assessment: Stomach aches were consistent with a known side effect of guanfacine. Dr. Hobbs recommended reducing to once daily dosing temporarily to allow accommodation. Plan: Drop guanfacine back to 1mg once daily for now. Plan to attempt dose increase again once GI symptoms improve. Medications: - Guanfacine Reduced from 1mg twice daily to 1mg once daily (adjust) Key clinical quotes: "It was totally ok to start twice a day, but I might drop back to once a day for now and then we can try an increase soon if the symptoms improve."
