Narrative: SPRUCE MESSAGE VISIT Date: 2025-11-13 Encounter: Spruce text message CC: Parent requesting updated dexamethasone prescription for recurrent croup management. HPI: Theo has a documented history of recurrent croup for which he has previously been prescribed dexamethasone. The parent requested a new prescription to have on hand, noting the existing prescription had expired. The prior prescription on file was for two 2mg tablets; the parent later clarified that four 2mg tablets had been filled from a prior physician. Dr. Hobbs had sent a new prescription for 6mg tablets, which encountered a pharmacy conflict with the existing 2mg script. Assessment: Theo has a known history of recurrent croup requiring on-demand dexamethasone. The pharmacy conflict appeared to stem from a pre-existing prescription for 2mg tablets interfering with the newly sent 6mg tablet prescription.
