Narrative: SPRUCE MESSAGE VISIT Date: 2025-12-03 Encounter: Photo review (photos of leg lesions sent by parent) followed by Spruce text message CC: Recurring skin infection on leg with multiple new red spots, possibly molluscum with secondary bacterial infection. HPI: Theo presented with a skin infection on his thigh that had been ongoing for several weeks, initially beginning as an irritated mole. The parent had been managing with daily cleaning, Neosporin, and bandaging with partial improvement and scabbing over. After the scab came off, the infection recurred and several additional similar red spots appeared on the same leg. By December 6th, a separate smaller lesion on the back of his leg had become painful, hard at center, and increasingly red despite two days of topical antibiotic treatment. Assessment: Dr. Hobbs assessed the lesions as likely molluscum with one lesion that was more prominently infected. The secondary lesion that worsened by December 6th was assessed as definitively infected but not severe, with recommendation for hot packing to facilitate drainage. Plan: Prescription antibiotic sent to Walgreens on Vernon. For the worsening secondary lesion, hot packing was recommended to bring it to a head. Parent reported successful drainage and healing by December 8th.
