Narrative: SPRUCE MESSAGE VISIT Date: 2026-03-06 CC: Persistent diaper rash management and 6-month well-child visit with vaccine planning HPI: Daisy is approximately 6-7 months old and presenting for a well-child check that was previously missed due to scheduling issues. She has an ongoing diaper rash that the parent reports is improving but slowly. Parent has been applying Aquaphor. Dr. Hobbs also discussed the 6-month vaccine schedule at this time. Daisy is behind on vaccines as the family missed a visit over the holidays due to influenza A illness. Encounter: In-office visit was initially missed on 3/6 due to a provider scheduling conflict; appointment was rescheduled to Monday 3/9 at 8:30am at Wayzata. In the interim, Dr. Hobbs recommended an OTC compounded topical mixture for the diaper rash: equal parts Aquaphor, Balmex, and Lamisil applied liberally. Vaccines discussed for the upcoming visit included DTaP, Hib, PCV, Flu, and Rotavirus. Parent prefers to administer one vaccine at a time to monitor for reactions.
