Narrative: SPRUCE MESSAGE VISIT Date: 2023-12-01 Encounter: Spruce text message followed by in-person home visit CC: Flu vaccine administration for a child with significant needle phobia. HPI: Dobson is a pediatric patient with a known history of significant anxiety around shots. Following an in-person visit on December 1, 2023, arrangements were made to administer the flu vaccine. The Hy-Vee FluMist option was explored but deemed unreliable, so Dr. Hobbs offered to make a home visit on December 3 to administer the vaccine in a familiar, low-stress environment. Assessment: Annual flu vaccination indicated; needle phobia noted. Home visit arranged to minimize anxiety and optimize successful vaccine administration. Plan: Dr. Hobbs made a home visit on December 3, 2023 and administered the flu shot. Parent reported this was the best Dobson had ever done with a shot.
