Narrative: SPRUCE MESSAGE VISIT Date: 2025-05-07 Encounter: Spruce text message CC: Fever of 102°F and sore throat with concern for strep throat given school exposure. HPI: Dobson presented with a fever of 102°F and sore throat, prompting early school pickup. The school had notified the parent that strep throat was circulating. Upon parent assessment after pickup, Dobson appeared only mildly subdued, reported the sore throat was not severe, and had predominantly nasal congestion. Throat appeared visually normal with no erythema or exudates. Children's ibuprofen was given at home. Assessment: Presentation most consistent with a viral upper respiratory illness. Strep throat considered less likely given normal throat appearance (no erythema, no exudates) and predominance of nasal congestion. Strep testing deemed unnecessary at this time. Plan: No strep testing recommended. Advised rest, fever control, and increased fluid intake. Parent instructed to monitor for worsening or symptoms lasting more than 3 days and to follow up with Dr. Hobbs if concerned.
