Narrative: SPRUCE MESSAGE VISIT Date: 2025-08-26 Encounter: Spruce text message followed by in-person office visit CC: Tdap and meningococcal vaccines needed for school enrollment. HPI: Dobson required Tdap and meningococcal vaccines that were identified as necessary for school enrollment. The parent had not realized these were required and requested a timely appointment. Dobson has a known history of significant anxiety around injections. The appointment was scheduled for August 29, 2025 at the Edina office. Assessment: Routine adolescent vaccines (Tdap and meningococcal) indicated for school requirements. Child with needle phobia managed successfully in-office. Plan: In-person office visit completed August 29, 2025. Both required vaccines administered. Vaccination records sent to family for school submission.
