Phone Visit
Narrative: SPRUCE MESSAGE VISIT Date: 2024-08-22 Encounter: Phone call via Spruce followed by Spruce text messages CC: Severe headache, fever, vomiting, rash, and neck stiffness while on vacation, with concern for meningitis. HPI: Eamon, approximately 12 years old, was on vacation when parents reported he had been restarted on Guanfacine 1mg approximately 10 days prior and was experiencing exhaustion and mild headaches attributed to re-acclimation to the medication. On 2024-08-22, he developed a severe headache that did not respond well to ibuprofen, preventing sleep (only 2 hours), along with a facial rash/red cheeks (similar presentation to Fifth Disease, which his brother had one week prior), nausea, vomiting, and subjective fever (no thermometer available). Neck stiffness was also reported. Parents expressed concern for meningitis. Assessment: Clinical picture consistent with a viral illness, likely enterovirus or parvovirus (Fifth Disease), given rash, fever, headache, and sibling exposure. Bacterial meningitis considered but felt less likely given clinical context; Dr. Hobbs advised that humans with bacterial meningitis look and act very sick.
