Narrative: SPRUCE MESSAGE VISIT Date: 2024-02-22 Encounter: Spruce text message CC: Heart murmur detected incidentally at CVS during strep test visit. HPI: Penelope, born 08/28/2017 (age approximately 6), was taken to CVS for a strep test when a staff member reported hearing a heart murmur — something that had not been previously detected including at recent dermatology appointments. Parents were concerned the examiner may have been rushed and wanted a formal evaluation. No symptoms of cardiac disease were reported. Assessment: Dr. Hobbs reassured the family that incidental quiet murmurs are very common in children and often appear only intermittently. He offered to help sort it out with an in-person evaluation. Plan: In-person evaluation scheduled for early the following week to assess the murmur. Key clinical quotes: "It's super common for kids to have quiet murmurs that show up only sometimes. Happy to help sort that out." Source: Spruce conversation t_27C02FVS44O00
